摘要:
目的 了解臨滄地區6個月~5歲健康兒童末梢血各項血細胞檢測參數的參考值范圍,為本地區兒童保健及臨床診斷提供參考。方法 使用ABXPentra MS60全自動五分類血球分析儀及原裝配套試劑對2018年1月~10月到我院兒童保健科健康體檢的兒童進行末梢血血細胞檢測,將檢測的原始數據通過瑞美軟件導入Excel,利用Excel篩選出6個月~5歲的兒童3356例,分別比較不同年齡段男女兒童WBC、RBC、HGB、HCT、MCV、MCH、MCHC、PLT 8個項目的參考值范圍。結果 ①5歲以下兒童WBC、PLT隨年齡的增加而降低,HGB、HCT、MCV、MCH 、MCHC隨年齡的增加而增加,RBC的變化不明顯;②6月~1歲時,不同性別兒童RBC、HGB、MCV、MCH、MCHC、PLT比較,差異有統計學意義(P<0.05),WBC、HCT比較,差異無統計學意義(P>0.05);2~4歲時,不同性別兒童8項指標中大多數基本一致,僅個別指標存在差異;4~5歲時,不同性別兒童WBC比較,差異有統計學意義(P<0.05);不同性別兒童MCV、MCH在所有年齡段中比較,差異均有統計學意義(P<0.05);③同性別兒童不同檢測項目在相鄰兩個年齡段間比較均存在一定的差異性,其中2~3歲與3~4歲兒童所有檢測項目比較,差異均有統計學意義(P<0.05)。結論 臨滄地區5歲以下健康兒童血細胞檢測參數因性別、年齡的不同,應該制定獨立的血細胞分析醫學參考值范圍,為本地區的兒童保健提供更為精準的醫療保健服務。
Abstract:
Objective To understand the reference range of blood cell detection parameters of peripheral blood in healthy children from 6 months to 5 years old in Lincang area, and provide reference for children's health and clinical diagnosis in this area.Methods The ABXPentra MS60 automatic five-class blood cell analyzer and original matching reagents were used to detect peripheral blood cells in children who were admitted to the Children's Health Department of our hospital from January to October 2018. The raw data were imported into Excel through Ruimei Software. 3356 children aged 6 months to 5 years old were selected by Excel to compare the reference range of WBC, RBC, HGB, HCT, MCV, MCH, MCHC and PLT for boys and girls of different ages.Results ①The WBC and PLT of children under 5 years old decreased with age, HGB, HCT, MCV, MCH, MCHC increased with age, and the change of RBC was not obvious;②The difference of RBC, HGB, MCV, MCH, MCHC and PLT in children of different genders was statistically significant at 6 months to 1 year old (P<0.05). There was no significant difference between WBC and HCT (P>0.05). At the age of 2~4 years, most of the 8 indicators of children of different genders were basically the same, only some indicators were different; when 4~5 years old, the WBC of different genders was statistically significant (P<0.05); different genders Children's MCV and MCH were statistically significant in all age groups (P<0.05).③There were some differences in the different test items of children of the same sex in the adjacent two age groups. The differences between the two groups of children aged 2~3 years and 3~4 years old were statistically significant (P<0.05).Conclusion The blood cell detection parameters of healthy children under 5 years old in Lincang area should be independent of the gender and age. The medical reference value range of blood cell analysis should be established to provide more accurate medical care for children's health care in the region.
